The University of Rhode Island uses a procedure of self-managed application materials. This means that the applicant is responsible for gathering all of the application materials together (with the exception of the official GRE scores) into one packet and sending the packet to The Physical Therapy Department at the address below.
GRE Scores:
The applicants should arrange for their scores from the Graduate Record Exam (GRE) to be sent directly to the Physical Therapy Department .
Forms:
Applicants are required to complete URI's Graduate School Online Application.
Applicants must also fill out the two online PT specific forms found here. Once the applicant submits the online prerequisite form, he or she will be sent an username and password to the email address indicated, allowing access to secure PT Department Intranet areas of the website and monitoring of their application status
In addition, applicants must arrange to have sent to the Physical Therapy Department at least two signed personal reference forms (at least one of which must be from a PT). These forms are in *.pdf, and can be downloaded and printed for the referee to fill out, sign, and mail. We recommend that along with the reference forms, the applicant supply the referee with an addressed, stamped envelope for mailing.
The status of the PT Department receiving these form can be monitored by use of the PT Department Intranet.
